In order to make your reservations, the country of Perú requires all travel companies to secure reservations with details from your passport and the passports of your travel companions.
Please use the form to submit your travel plans, including the following information:
- Full Name as it appears on your passport
- Passport Number
- Date of Birth
- Issuing Country
Important to note – At the time of travel, your passport will need to have no less than 6 months time before expiration. For instance, if you plan to travel to Perú, June 15, 2019 and your passport expires September 15, 2019, you will not be allowed to enter the country. Please double check your expiration date and plan accordingly.
